Circumstances:

Died while in MIA status. Stationed on a steam merchant ship southeast of Guantanamo Bay that was struck by a torpedo, which caused her cargo (4000 tons of explosives) to explode. The ship completely disappeared in 30 seconds, and the blast was so large that a ship 40 miles away witnessed it.
