Circumstances:
On April 19, 1968, an aircraft from B Company, 229th Assault Helicopter Battalion, 1st Air Cavalry Division, was the lead ship on a combat assault into A Shau Valley. The helicopter was damaged by an enemy grenade, causing the tail boom to separate and the aircraft to crash. Aircraft commander WO1 Charles J. Harrington and pilot 1LT David J. Nesset suffered fatal injuries in the crash.
